El Plan de Calidad de la Consejería de Salud de la Junta de Andalucía, describe como se produce este encuentro en un espacio compartido en el que pacientes y profesionales comparten valores, responsabilidades, conocimientos y decisiones. Durante los últimos diez años, los Planes de Calidad han traí- do un cambio cultural y estratégico que ha propiciado la orientación del Sistema Sanitario Público Andaluz (SSPA) hacia el ciudadano. Este hecho ha dado pie a que, durante este periodo, se hayan desarrollado numerosas estrategias que han permitido disponer de la mayoría de los elementos que permiten avanzar rápidamente hacia un modelo de exce lencia en la gestión de las enfermedades crónicas. El objectivos incluye: 1) Mejorar la calidad de vida de los pacientes con enfermedades crónicas reduciendo la discapacidad y la dependencia y favoreciendo un mayor nivel de autonomía funcional y social 2) Propiciar la responsabilidad y la solidaridad social e individual para la mejora del estado de salud de los ciudadanos ayudando a la participación activa de la ciudadanía y evitando las inequidades 3) Mejorar la adecuación de la organización asistencial y la oferta de servicios a las necesidades del paciente con enfermedades crónicas.

The Andalucian Plan for Integrated Care to Patients with Chronic Diseases does not intend to redefine a new strategy for all chronic diseases, but that it aspires to be a complementary strategy to align efforts in the different plans and strategies of the SSPA. Despite the efforts made during the last years, the loss of the continuity of care between the areas of primary and hospital care, as well as between health and social care remains one of the major problems for patients with chronic diseases, and especially for those persons with concurrent diseases. The objectives include: 1) Improve the quality of life of patients with chronic diseases, reducing disability and dependency and promoting a higher level of functional and social autonomy 2) Promote responsibility and individual and social solidarity for the improvement of the health status of citizens, helping to foster active citizenship and avoid inequalities 3) Improve the adequacy of the health care organization and the offer of services to the needs of the patient with chronic diseases.
