Ce document est l'aboutissement d'une démarche issue d'une collaboration du ministère de la Santé et des Services sociaux et de 15 autres ministères et organismes.Ce plan d'action engage le gouvernement et ses partenaires, en conformité avec les orientations fondamentales de la Politique nationale de lutte à l’itinérance – Ensemble, pour éviter la rue et en sortir, à mettre en œuvre des actions concrètes destinées à prévenir et à réduire l'itinérance. Ces actions s'inscrivent en réponse aux orientations identifiées dans chacun des axes d'intervention prioritaires de la Politique.

This document is the culmination of an approach resulting from the collaboration of the Department of Health and Social Services and 15 other departments and agencies.
This action plan commits the government and its partners, in accordance with the fundamental orientations of the National Policy to Combat Homelessness - Together, to Avoid the Street and Get Out of It, to implement concrete actions intended to prevent and reduce homelessness. These actions are in response to the orientations identified in each of the priority areas of intervention of the Policy.
